Examining the Occurrence and Clinical Impact of Difficult Experiences that Emerge during a Mindfulness-Based Intervention among Individuals at High-Risk of Suicide.

Among adults at high risk of suicide who took part in a mindfulness-based therapy, 18% experienced distress, dysregulation, or agitation during practice, most often anxiety or hallucinations. Those who had such difficulties showed some diagnostic differences at baseline but were not at significantly greater risk of a suicidal event or psychiatric hospitalization over a 12-month follow-up. No suicide attempts occurred among the 9 participants who experienced mindfulness difficulty, compared to 5 attempts among those who did not. The findings suggest that although difficulties during mindfulness are common, they do not increase the risk of serious clinical outcomes in this high-risk population.

Attentional Control May Be Modifiable with Mindfulness-Based Cognitive Therapy to Prevent Suicide

A randomized clinical trial tested whether Mindfulness-Based Cognitive Therapy to Prevent Suicide (MBCT-S) improves attentional control, an objective marker of suicide attempt, in 135 high suicide risk Veterans. Seventy Veterans received MBCT-S plus enhanced treatment as usual (eTAU), and 65 received eTAU only. Computerized Stroop and emotion Stroop tasks were administered three times over six months. Veterans receiving MBCT-S showed a more favorable trajectory of attentional control over time, with improved combat-stress interference scores. Interference processing time for negative affective words deteriorated over time among Veterans receiving eTAU only. The authors suggest MBCT-S may effectively target attentional control in high suicide risk Veterans.
