A Delphi consensus study with 89 experts from 17 countries identified 30 extra-pharmacological variables that are important or very important for reporting the setting in psychedelic clinical trials. These variables, forming the ReSPCT guidelines, cover physical environment, dosing session procedure, therapeutic framework and protocol, and subjective experiences. The findings reveal significant ambiguities in current conceptualizations of set and setting. The guidelines provide a new standard for designing and documenting contextual factors in psychedelic research.

Psilocybin and other serotonergic psychedelics are used in research settings with safety measures including controlled environments, staff presence, screening, and psychoeducation. An analysis of study materials from psilocybin trials over the past two decades found that psychoeducation documents varied but commonly emphasized biological and physical safety, psychological safety and well-being, aspects of setting, and the potential for expectancies. The materials prioritized biological and psychological safety across all sites. The authors also identified elements unrelated to safety that may contribute to participant expectancies and suggest these extrapharmacological factors be studied systematically to maximize safety while minimizing extraneous expectancies.

Psilocybin, a psychedelic compound, was generally well-tolerated and reduced obsessive-compulsive disorder (OCD) symptoms in a small clinical trial. No serious adverse events, psychotic symptoms, or changes in suicide severity occurred. Psilocybin, but not placebo, significantly lowered scores on the Yale-Brown Obsessive Compulsive Scale. After eight weeks of treatment including at least four high doses, 73.3% of participants responded (at least 35% reduction in symptoms), and 40% achieved remission. Benefits diminished but remained substantial at six months. Higher cumulative doses were linked to greater symptom reduction. Larger trials are needed to confirm efficacy and refine protocols.

In the first year of Oregon's regulated psilocybin program, 5,935 clients participated in 5,375 sessions. Utilization peaked in the second quarter before stabilizing. About a third of participants lived outside Oregon. The largest age group was 35–49; women and LGBTQ+ individuals were well represented, but racial diversity was limited, with White participants making up 84–92% quarterly and African American participation at 2.1%. Adverse events were rare, with behavioral and medical rates of 2.42 and 2.79 per 1,000 sessions. The program serves as both a wellness modality and an alternative for mental health distress, though racial disparities and socioeconomic barriers persist.

Obsessive-compulsive disorder (OCD) is a common and disabling condition for which current treatments are often insufficient, making alternative approaches a priority. Psilocybin may offer a safe and effective treatment avenue. This chapter reviews OCD symptoms, epidemiology, and disease mechanisms, along with existing treatments and their limitations, as background for discussing psilocybin and similar compounds. It covers psilocybin's mechanisms of action relevant to OCD and describes the authors' experiences with incidental observations, prospective research, and ongoing explorations of psilocybin for OCD.
