Abstract
Background/context: Anxiety disorders are common in community mental health services and are frequently maintained by worry, rumination, threat monitoring, reassurance seeking, avoidance, and maladaptive attempts to control internal experience. Aim/objective: This perspective chapter examines Detached Mindfulness as a metacognitive technique for anxiety disorders and discusses its clinical translation within an Italian community mental health service. Approach: The chapter integrates foundational Metacognitive Therapy literature, recent evidence on metacognitive mechanisms and Detached Mindfulness, selected mindfulness research, and preliminary practice-based clinical material previously published as a short conference report. Key insights: Detached Mindfulness is best understood as a technique that helps patients notice thoughts, images, and sensations as mental events while reducing engagement with worry, analysis, checking, and reassurance seeking. In public mental health care, its use should be embedded in assessment, risk evaluation, multidisciplinary planning, and outcome monitoring. Conclusions/implications: Detached Mindfulness may be feasible and acceptable as a brief, theory-driven, Metacognitive Therapy-informed component of routine care, but current evidence does not establish efficacy or comparative effectiveness across anxiety disorders. The chapter contributes to Mindfulness Across Disciplines by clarifying conceptual boundaries between mindfulness-related capacities, metacognitive theory, clinical implementation, and community psychiatry.
