Psilocybin, a psychedelic compound, reduces the brain's ability to synchronize its electrical activity at 40 Hz in response to auditory clicks. In a double-blind, placebo-controlled crossover study, 20 healthy volunteers received either psilocybin (0.26 mg/kg) or placebo. Measurements taken before and after ingestion showed that psilocybin decreased the phase-locking index and amplitude of the 40 Hz auditory steady-state response, and the degree of this reduction correlated with changes in cognition and affect. These findings support the role of gamma oscillations in cognitive processing and their disruption in psychosis.

Most patients with treatment-resistant depression reported positive experiences with esketamine nasal spray. In a survey of 236 outpatients, 88.4% reported enhanced quality of life. Satisfaction levels varied: 10.2% were unsatisfied, 19.1% partially satisfied, 44.4% satisfied, and 26.3% very satisfied. The most satisfied patients noted early improvements in depressed mood, suicidal thoughts, and restlessness, and reported functional gains across all domains. Artificial intelligence analysis of open-ended responses identified themes of personal growth and a desire for tailored treatment settings. Integrating patient-reported experiences may help personalize care and improve adherence.

Brief app-based mindfulness and socio-emotional dyadic interventions improved depression, anxiety, emotion regulation, and stress recovery only among people who had shown resilient stress-reactivity profiles during the COVID-19 pandemic. Those with vulnerable profiles—characterized by less plasticity in stress recovery—did not improve on any outcome. The findings come from 253 community adults who used a daily 12-minute app for 10 weeks with weekly coaching. The results suggest that low-dose digital mental interventions may benefit individuals with greater stress resiliency, while more vulnerable individuals may need more intensive or personalized approaches.

For 70 years depression treatment was limited by the monoamine hypothesis until the approval of the S-enantiomer of ketamine, an NMDA receptor blocker with rapid antidepressant and antisuicidal effects. Dextromethorphan, another NMDA receptor antagonist, has also been approved for depression in combination with bupropion. The GABA-A receptor modulator brexanolone has shown relatively rapid antidepressant efficacy. However, clinical utility is compromised by high drug costs, mandatory monitoring requirements, parenteral administration, lack of insurance coverage, COVID-19 effects on healthcare systems, and psychopharmacology training gaps. This narrative review analyzes the clinical pharmacology of recently approved antidepressants and barriers to bench-to-bedside translation. Overall, meaningful advances have not reached many depressed patients, including those with treatment-resistant depression who might benefit most.

Dissociative disorders involve disruptions in memory, consciousness, perception, motor control, and identity. This systematic review of 13 functional neuroimaging studies, conducted on 51 patients with dissociative identity disorder (DID), 28 with depersonalization disorder, 24 with dissociative amnesia, and 6 with other or unspecified dissociative disorders, identifies common brain activation patterns. Prefrontal cortex dysfunction appears prominent across these disorders. In DID, changes in the caudate's neural network relate to altered identity states, and dysfunction of the anterior cingulate gyrus also plays a role. Other affected regions include parietal, temporal, and insular cortices, along with subcortical areas. Further research is needed to clarify neurofunctional correlates for better tailored treatments.
