Modulation of neural complexity and consciousness in temporal lobe seizures: Effects of high-frequency pulvinar stimulation.

Loss of consciousness during temporal lobe seizures is linked to reduced brain signal complexity and abnormal synchronization in the thalamocortical system. The medial pulvinar nucleus (PUM) is involved in seizure spread and awareness impairment. In eight patients with drug-resistant temporal epilepsy, high-frequency PUM stimulation during hippocampus-induced seizures attenuated the decline in neural complexity measured by permutation entropy. Reduced entropy alterations correlated with improved awareness scores on the Consciousness Seizure Scale. The findings suggest that PUM stimulation preserves neural complexity and may help maintain consciousness and cognitive function during seizures, though larger studies are needed.

Epilepsy and Ecstatic Experiences: The Role of the Insula.

Ecstatic epilepsy is a rare form of focal epilepsy where the seizure aura is a blissful state of mental clarity and certainty, often described as religious or mystical. Over the last decade, evidence has located the symptomatogenic zone for this experience in the dorsal anterior insula. The neurocognitive hypothesis proposes that the feeling of mental clarity arises from suppression of interoceptive prediction errors and unexpected surprise from internal or external signals, which are normally processed by the dorsal anterior insula. This mimics a perfect prediction of the world, inducing certainty. Ecstatic epilepsy thus serves as a model for the anterior insula's role in uncertainty and surprise.
