Current medical ethics for organ harvesting under anesthesia rely on the untested assumption that no behavioral response means no consciousness. This paper argues that anesthesia may suppress the narrative, left-hemispheric system (Type 2 consciousness) while leaving direct, pre-verbal, right-hemispheric awareness (Type 1 consciousness) partially intact. Evidence from near-death experiences, coma studies, psychedelic neuroscience, flow states, and contemplative neuroscience shows that the narrative self can disappear while direct awareness continues. The authors do not claim organ donors feel pain but that a form of registering awareness may be present undetectable by current instruments. They call for neuroimaging research on right-hemispheric activity during anesthetic states in organ donation.

Western civilization has gradually moved away from understanding what consciousness actually is, not through ill intent but through accumulating assumptions. Consciousness is not a product of brain activity, quantum processes, or information integration; it is a state of being in which a living organism directly processes environmental requirements using natural means. The Default Mode Network, when dominant, disrupts this natural state, and mental suffering maps onto self-reinforcing DMN patterns. Independent sources—indigenous traditions, medieval mysticism, developmental psychology, and contemplative neuroscience—converge on a single underlying reality, offering a map for returning to this original state.

Scientific research on consciousness from quantum physics, neuroscience, developmental psychology, anesthesia, near-death experiences, and psychedelics provides fragments of understanding, each observed from a specific position with inherent blind spots. This paper presents major findings without citing authorities, describes the position from which each observation was made and its limitations, and contrasts these with what becomes visible through direct, unfiltered first-person awareness. The aim is to produce a complete picture that convinces on its own.

The brain does not generate consciousness but instead translates raw sensory data into a narratively structured, culturally contextualized experience. The right hemisphere acts as a persistent, pre-narrative receiver that continues to register sensory information even when left-hemispheric narrative processing is suppressed by anesthesia, coma, near-death experiences, or psychedelics. This framework explains intraoperative awareness, implicit memory during unconsciousness, universal features of near-death experiences alongside cultural variation, sensory registration in comas, and the quality of psychedelic experiences when the Default Mode Network is suppressed. A three-layer model is proposed: sensory organs as autonomous receptors, the right hemisphere as a raw-data receiver, and the left hemisphere as a narrative translator.
