| Key findings | The authors argue that clinical evidence for psilocybin-assisted therapy in palliative and serious-illness populations is accumulating rapidly, citing randomized trials in cancer-related existential distress with sustained reductions in depression and death anxiety and real-world Oregon cohort data showing symptom improvement with low serious-adverse-event rates. They contend that eligibility should rest on individualized clinical assessment rather than systematic exclusions, paired with funding mechanisms so access does not depend on wealth alone. |
Abstract
AbstractBackground. Existential distress and treatment-resistant depression are common and difficult to manage in patients with serious and life-limiting illnesses. Psilocybin-assisted therapy has shown early promise for these conditions, but access outside research settings remains narrow, inconsistent, expensive and often shaped by outdated eligibility rules. These include extremely limited lists of diagnosis- or just blanket age-based cut-offs, that are not grounded in evidence specific to palliative populations. Regulators slowly adjust to the state-of-the-art knowledge generated by psilocybin-assisted therapy, and overall, often present poor understanding of palliative populations specific needs.Objective. To review current and emerging clinical evidence for psilocybin-assisted therapy in serious and life-limiting illness, highlighted with the examples of the EU “Horizon Europe” funded PsyPal project and a Johns Hopkins-led trial in amyotrophic lateral sclerosis (ALS), and to examine the regulatory, informational, and financial barriers that shape access for palliative populations.Findings. Randomized trials in cancer-related existential distress report sustained reductions in depression and death anxiety. Furthermore, a majority of participants in the founding psilocybin studies rated their session among the most meaningful experiences of their life. Project PsyPal is currently the first randomized trial of psilocybin therapy in non-oncology palliative populations, enrolling patients with COPD, ALS, multiple sclerosis, and atypical parkinsonian disorders across four European sites with no upper age limit; a separate trial at Johns Hopkins is testing psilocybin for mood and hopelessness in ALS specifically. Real-world cohort data from Oregon's regulated program corroborate meaningful symptom improvement and low serious-adverse-event rates. Access, however, remains constrained by diagnostic and research-representation gaps, high out-of-pocket cost, and inconsistent regulatory pathways, illustrated by a Canadian patient refused psilocybin for existential relief who was approved for medical assistance in dying within weeks.Conclusion. Clinical evidence relevant fort to palliative and serious-illness populations is accumulating rapidly. Eligibility frameworks for psilocybin-assisted therapy in palliative care should be built on individualized clinical assessment rather than systematic exclusions, paired with funding mechanisms that prevent access from becoming a function of wealth alone.
