Dreaming arises from the same hierarchical predictive processing that governs waking cognition, but with key modifications: lack of sensory and motor input and a predominance of associative, non-rational primary process thinking. Emotional needs guide behavior via a value system generating pleasure and unpleasure, and the brain constantly updates its predictions to minimize prediction error. Repressed priors—mental events that cannot be reconsolidated despite ongoing error signals—correspond to conflictual complexes and may become accessible in symbolic form during dreams and psychedelic states. Evidence from neuroimaging supports this framework, and an ongoing trial with stroke patients who lost the ability to dream tests whether dreaming is necessary for intact sleep architecture and memory consolidation.
Transformation of manifest dreams, analyzed with a precise empirical coding system, may serve as a promising indicator of therapeutic changes in unconscious mental functioning during psychoanalysis. The Zurich Dream Process Coding System (ZDPCS), grounded in a dream generation model by Moser and von Zeppelin that integrates interdisciplinary sleep and dream research, provides a valid and reliable method for this analysis. Two dreams from a severely traumatized, chronically depressed patient—one from the beginning and one from the third year of the LAC Depression Study—were analyzed using the ZDPCS. The authors hypothesize that dream transformation measured by this system results from memory processes involving traumatic embodied memories during dreaming.
For chronically depressed patients with early trauma, intensive psychoanalysis can produce lasting structural change by transforming traumatic embodied memories within the analytic relationship. A longitudinal case from the LAC Depression Study illustrates how high-frequency psychoanalysis enabled a patient to discover and understand embodied memories of early trauma through the transference relationship, with changes in dream processes—measured by the Zurich Dream Process Coding System—showing a shift from helplessness-dominated nightmares to dreams with greater affect regulation, symbolic capacity, and self-agency. These transformations persisted more than a decade after treatment termination. The article integrates psychoanalytic theory, dream research, and neurobiological models of memory reconsolidation to explain how psychoanalysis uniquely fosters sustained therapeutic change in this treatment-resistant population.
A literature review on psilocybin-assisted psychotherapy for mental illnesses examined peer-reviewed studies from 2017 to 2022. Nine studies covered tobacco addiction, anxiety and depression related to life-threatening cancer, and treatment-resistant depression. Rapid clinical improvement of various symptoms was observed, with the strongest evidence for treating tobacco addiction and anxiety and depression tied to life-threatening illnesses. No serious adverse events were reported. However, current studies have limitations, including small sample sizes, difficulties with blinding, and a non-representative treatment population. The results are not representative but provide indications of effective treatment and serve as a starting point for further studies.
