Mindfulness training applied to addiction therapy: insights into the neural mechanisms of positive behavioral change

Addiction arises from an imbalance between bottom-up reward-learning processes and top-down executive control in the brain, with drug use weakening prefrontal cognitive control networks and hijacking striatal circuits meant for natural rewards. Mindfulness-based interventions (MBIs) can be seen as mental training that exercises and strengthens these brain networks. This review proposes that MBIs remediate addiction by regulating frontostriatal circuits, restoring balance between top-down and bottom-up processes. Evidence indicates MBIs improve cognitive control over drug-related automaticity, attentional bias, and cue reactivity, while increasing responsiveness to natural rewards. An integrative account of neural mechanisms is offered to inform addiction therapy.

Mindfulness-Oriented Recovery Enhancement Reduces Pain Attentional Bias in Chronic Pain Patients

A mindfulness-oriented intervention called Mindfulness-Oriented Recovery Enhancement (MORE) reduced pain attentional bias (AB) among chronic pain patients receiving opioid analgesics. Prior to treatment, participants showed a significant bias toward pain-related cues presented for 2,000 milliseconds but not for 200 milliseconds. After an 8-week program, those in MORE showed significantly reduced pain AB compared to their pretreatment levels, while a social support group showed no change. Decreases in pain AB were linked to increased perceived control over pain and less reactivity to distressing thoughts and emotions. The authors propose MORE as a nonpharmacological approach for addressing factors linked to chronic pain.
