Awareness during general anesthesia—the postoperative recall of sensory experiences while under anesthesia—remains among medicine’s most haunting yet underexplored phenomena. Affecting 1 to 2 in every 1,000 patients, anesthesia awareness challenges assumptions of total unconsciousness and highlights the gap between scientific understanding and lived experience. Even with proper anesthetic practices, episodes of intraoperative awareness can occur, sometimes without pain. However, many report vivid sensations, overheard surgical conversations, and intense fear or a sense of impending doom. This chapter weaves together the personal narratives of two family members—a mother and brother—who endured rare, traumatic anesthesia awareness events, as well as my own experience with a medically-induced anesthesia overdose. These stories illustrate the significant psychological and neurobiological consequences of anesthesia awareness, such as post-traumatic stress, hypervigilance, and diminished trust in healthcare.
