Slow pranayama, a yogic breathing technique considered a form of meditation, shows promise for reducing anxiety symptoms by modulating heart rate variability (HRV). Anxiety disorders, among the most common mental health conditions, are linked to autonomic nervous system dysregulation and decreased HRV, a biomarker of vagal tone and resilience. Slow breathing with a prolonged expiratory phase enhances vagal tone, restores sympathovagal balance, and promotes homeostasis. This review explores the therapeutic potential of slow pranayama in managing anxiety disorders, examining current evidence, limitations, and future directions for integrating yoga-based practices into mental health care.
Sleep paralysis is a state during falling asleep or waking up in which a person is conscious but unable to move due to muscle atonia. It is often accompanied by three types of hallucinations: the intruder, the incubus, and out-of-body experiences, which can cause intense fear and distress. Sleep paralysis co-occurs with chronic conditions such as hypertension and obstructive sleep apnea, and is associated with post-traumatic stress disorder. Folklore has depicted the phenomenon for centuries, creating superstitions. Primary care physicians should empathetically address patient concerns, dispel myths scientifically, and consider asking about sleep paralysis episodes during routine interviews to liaise with specialists when needed.
A 52-year-old Indo-Caribbean man with obstructive sleep apnea experienced recurrent fearful isolated sleep paralysis. Recurrent sleep paralysis is often under-reported but can cause significant distress, including bedtime anxiety, bedroom avoidance, and poor sleep quality, leading to a vicious cycle of sleep deprivation. In this case, the patient's sleep paralysis was cured by continuous positive airway pressure treatment for his obstructive sleep apnea. The authors aim to raise awareness so patients seek help and researchers can better understand the condition.
Kapalbhati, a fast-paced breathing exercise from yoga, produces significant changes in heart rate variability (HRV) during and after practice, indicating a shift away from parasympathetic (rest-and-digest) activity and toward sympathetic (fight-or-flight) activation. Brain wave changes were not statistically significant compared to baseline. After the exercise, gamma wave activity increased, suggesting altered control of the brain's default mode network, which is linked to self-referential thought and mind-wandering. The findings clarify previous inconclusive results about kapalbhati's immediate neurological and autonomic effects.
