823 Apparent seizure activity in a 9 year old female presenting as hypnopompic hallucinations

A 9-year-old girl with a history of tonsillectomy, ADHD, and behavioral issues presented with excessive daytime sleepiness and hypnopompic hallucinations that occurred 2–3 times per week initially, then decreased to 2–3 times per month. Polysomnography revealed mild obstructive sleep apnea and, notably, spike-and-wave formations in central and frontal leads during NREM sleep and sleep-wake transitions. The case suggests that seizure activity may present as hypnopompic hallucinations, in addition to other causes such as narcolepsy or medication effects.
