Rhabdomyolysis, though rare, is a serious complication that can arise from various diseases, including poisonings, which are more common causes than trauma. This case report describes a patient who developed exceptionally severe rhabdomyolysis, with creatine kinase levels reaching 516,455 U/L, after ingesting a small dose of a novel psychoactive substance called "Alice in Wonderland." Laboratory analysis confirmed the substance using a validated LC-MS/MS method on a whole blood sample.
