The pineal gland, a photo-neuro-endocrine organ inside the brain, secretes serotonin, melatonin, and N,N-dimethyltryptamine (DMT). This narrative review synthesizes information on the gland's physiology, anatomy, histology, and histopathology, and its role in endocrine, neurological, and psychiatric conditions. Melatonin shows promising potential for therapeutic effects in immunity, while DMT appears to have a role in psychiatric pathology and potential therapeutic effects. Proper tumoral screening and diagnostic protocols are needed.
