Happy water overdose: A trending but fatal street drug

A synthetic drug cocktail called "Happy water" is gaining popularity in Southeast Asian nightlife scenes, packaged in sachets or disguised as instant coffee or beverage mix. Two patients were admitted with reduced consciousness after a farewell party at a nightclub; urine toxicology was positive for MDMA, methamphetamine, diazepam, and tramadol. The mixture's unpredictable composition, combining sedative, stimulant, and hallucinogenic elements, makes overdose diagnosis challenging and poses acute risks of collapse, overdose, and death. Proper history taking, recognition of toxidromes, and presumptive drug screening are needed for diagnosis, and management requires urgent multidisciplinary medical intervention.
