In a secondary analysis of a randomized trial, 98 patients with episodic migraine received either enhanced mindfulness-based stress reduction (MBSR+) or stress management for headache. Sleep quality improved significantly from baseline to postintervention (20 weeks) in both groups, but anxiety, depression, and stress showed no significant changes over time or between groups. Baseline psychosocial scores did not predict treatment response (a 50% reduction in headache frequency). Mediation analysis suggested a small indirect effect of 6% for sleep, indicating that modest sleep improvements may have contributed to the efficacy of MBSR+.
