Major Depressive Disorder is highly prevalent and disabling, affecting 15.5% of the Brazilian population. Its pathophysiology involves monoaminergic and receptor theories, though none are conclusive. Pharmacological treatments rely on the monoaminergic theory despite limitations. Refractory depression is particularly challenging. Psychedelic substances such as ayahuasca, ketamine, and psilocybin show promising results but require more investigation. This literature review synthesizes studies on psychedelics for refractory depression, consulting PubMed/Medline and SciELO from 2000 to 2023. Ayahuasca acts on serotonin receptors; ketamine rapidly increases glutamate levels and reduces suicidal ideation; psilocybin affects 5HT2A receptors. Significant limitations remain, and standardized protocols are lacking.
