Self-inflicted transorbital intracranial foreign body following ingestion of hallucinogenic psilocybin mushrooms.

A 21-year-old man ingested psilocybin mushrooms and then drove a wood-cased pencil through his upper eyelid and left orbit into the brainstem. Surgeons removed the pencil using fluoroscopy-guided catheterization and stenting. Although the eyeball itself was not damaged, the trauma caused progressive neurological decline and death. The case highlights a rare, fatal self-inflicted orbital injury from a psychoactive hallucinogen and describes the multidisciplinary medical and surgical management required.
