Sahaj Samadhi Meditation versus a Health Enhancement Program for depression in chronic pain: protocol for a randomized controlled trial and implementation evaluation

This protocol describes a planned hybrid effectiveness-implementation study of Sahaj Samadhi Meditation for adults with chronic pain, depressive symptoms, and long-term opioid therapy. The randomized controlled trial will compare meditation against a Health Enhancement Program in 160 participants, with depressive symptoms as the primary outcome and pain, function, opioid use, and quality of life as secondary outcomes. The primary endpoint is at 12 weeks, with a 24-week follow-up. A mixed-methods implementation evaluation will examine delivering meditation in a clinical setting by non-regulated teachers. The authors propose this design will inform effectiveness and future scalability.
