Clinical features associated with ADB-BUTINACA exposure in patients attending emergency departments in England.

ADB-BUTINACA, a synthetic cannabinoid receptor agonist, has recently emerged as a drug of misuse in England. Among 1279 participants in the IONA study, ADB-BUTINACA was detected in 10 patients (9 male, median age 45) presenting since February 2021. Clinical features of toxicity included reduced level of consciousness, respiratory and/or metabolic acidosis, seizures, confusion, and hallucinations. In three cases ADB-BUTINACA was the only substance detected; in seven others, co-use of opioids, benzodiazepines, cocaine, or pregabalin occurred. All patients recovered with supportive care, one requiring intubation and ventilation. Median hospital stay was 19 hours.
