Severe poisoning after self-reported use of 2-(4-iodo-2,5-dimethoxyphenyl)-N-[(2-methoxyphenyl)methyl]ethanamine, a novel substituted amphetamine: a case series.

A case series of ten adolescents (average age 17) who arrived at emergency departments after using the novel amphetamine derivative 25-I-NBOME reported that most had tachycardia (90%), hypertension (70%), agitation (60%), and hallucinations (50%). Two patients had status epilepticus, one had multiple brain hemorrhages and acute kidney injury, and three required emergency intubation. Seven of ten were admitted to intensive care and given intravenous benzodiazepines. The drug appears extremely potent, with a reported dose as low as 500 μg, raising overdose risk. This cluster documents serious harm from a newly identified recreational drug.
